Telehealth Nurse (Work From Home)

About the role

We are seeking a qualified Registered Nurse to provide telehealth services for an Australian NDIS provider supporting participants in regional and remote communities. This full‑time, work‑from‑home role offers the chance to make a meaningful impact on the health outcomes of some of Australia’s most vulnerable individuals.

Key responsibilities

• Conduct telehealth continence assessments for NDIS participants.
• Prepare detailed service summaries and provider reports.
• Collaborate with on‑the‑ground care teams to deliver coordinated care.
• Support provider reporting and documentation requirements.

Required profile

• Registered Nurse with current registration.
• Experience in telehealth and supporting remote or vulnerable communities.
• Clinical background in renal care/dialysis, cardiovascular conditions, diabetes complications, liver/kidney issues, wound care or amputations.
• Familiarity with NDIS, disability, aged care, community care or Aboriginal health services is highly regarded.
• Strong written communication for service summaries and participant notes.
• Compassionate, culturally aware and confident working remotely.
