About the role
We are seeking a licensed US‑registered nurse to join our Alabang team as a Care Coordinator. The role blends clinical expertise with patient‑centered coordination, delivering both telephonic and virtual assessments to support individuals across the care continuum.
Key responsibilities
- Conduct telephonic and virtual patient assessments to evaluate health status and needs.
- Develop, implement, and regularly update individualized care plans.
- Coordinate services among physicians, specialists, home‑health agencies, and other providers.
- Educate patients and families on treatment protocols, medication management, and self‑care strategies.
- Monitor patient progress, document outcomes, and promptly escalate clinical concerns.
Required profile
- Active USRN license.
- 6 months to 5 years of clinical experience in acute care, ambulatory care, home health, care management, or case management.
- Strong patient assessment and care‑coordination abilities.
- Excellent written and verbal communication and documentation skills.
