Registered Nurse – Telehealth (RN/LPN)

About the role

We are seeking experienced Registered Nurses (RN) and Licensed Practical Nurses (LPN) to join our remote care team. In this telehealth position, you will provide virtual assessments, education, and coordinated care to patients across acute, ambulatory, home‑health, and care‑management settings.

Key responsibilities

• Conduct telephonic and video patient assessments.
• Educate patients on their conditions, medications, and treatment plans.
• Develop and manage individualized care plans.
• Monitor patient progress and escalate clinical concerns when needed.
• Coordinate care among providers, payers, and community resources.
• Maintain accurate documentation in electronic health record (EHR) systems.
• Support patient engagement and ensure continuity of care.

Required profile

• Clinical experience in acute care, ambulatory care, home health, or care management.
• Ability to work independently in a remote environment.
• Strong communication skills for patient interaction.
• Knowledge of chronic disease management and preventive care.

Required skills

• Proficiency with EHR/EMR systems.
• Experience using telehealth platforms.
