About the role

ISTA Solutions is seeking a qualified PHRN to serve as a Clinical Coordinator (MDS) on a work‑from‑home basis. The role focuses on reviewing pre‑hospital patient documentation, ensuring compliance with standards, and supporting accurate Minimum Data Set (MDS) assessments.

Key responsibilities

• Complete and submit accurate MDS assessments for residents.
• Develop, update, and maintain individualized care plans.
• Ensure all documentation aligns with Medicare/Medicaid, state regulations, and pre‑hospital protocols.
• Collaborate with nurses, therapists, and interdisciplinary teams to improve patient care quality.
• Maintain precise data reporting to support reimbursement and regulatory compliance.

Required profile

• Current, valid PHRN license.
• Minimum 1 year of bedside or clinical experience.
• Experience with Minimum Data Set (MDS) assessments.
• Strong knowledge of pre‑hospital care standards and regulatory requirements.
• Resides within the Greater Manila Area and can work shifting schedules.
