Care Coordinator (LPN/RN) – Night Shift with 14th Month Pay

About the role

We are seeking a dedicated Care Coordinator (LPN/RN) to join a healthcare team in Alabang, Muntinlupa. Working the night (graveyard) shift, you will provide virtual and telephonic patient coordination, ensuring continuity of care and supporting positive health outcomes.

Key responsibilities

• Conduct telephonic and virtual patient assessments and ongoing monitoring.
• Develop, implement, and update individualized care plans based on clinical guidelines.
• Educate patients and families on conditions, treatments, medications, and self‑management strategies.
• Monitor patient progress, escalating clinical concerns when needed.
• Coordinate care across providers, payers, and community resources.
• Document all interactions accurately in electronic health records (EHR) and care‑management systems.
• Collaborate with clinical and operational teams to improve workflows and patient engagement.
• Utilize clinical judgment to triage patient needs and support timely interventions.

Required profile

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) with at least 1 year of direct clinical experience.
• Experience in acute care, ambulatory, home health, or care management settings.
• Prior involvement in care coordination for offshore or international patients.
• Strong understanding of hospital care‑coordination processes and clinical workflows.
• Excellent written and verbal communication skills for remote patient engagement.

Required skills

• Proficiency with Electronic Health Records (EHR) systems.
• Experience using telehealth tools.
• Familiarity with digital care platforms.
