Remote Registered Nurse (RN) – Chronic Care Management

About the role

We are seeking a compassionate Registered Nurse to join our remote team, providing chronic care management support to patients enrolled in our program. The RN will conduct outreach calls, develop personalized care plans, and help improve health outcomes.

Key responsibilities

• Make monthly outreach calls to patients in the Chronic Care Management (CCM) program.
• Collaborate with patients and providers to create and update individualized care plans.
• Monitor symptoms, identify risks, and escalate care when needed.
• Coordinate with primary care providers, specialists, and community resources.
• Educate patients and caregivers on disease management, medication adherence, and healthy habits.
• Maintain accurate patient records in the electronic health record (EHR) system.
• Ensure compliance with CMS guidelines for CCM and promote preventive care.

Required profile

• Valid RN license in the Philippines.
• Minimum 1 year of clinical nursing experience (chronic care, case management, or primary care preferred).
• Strong knowledge of chronic conditions such as diabetes, COPD, hypertension, and heart failure.
• Excellent communication, critical thinking, and organizational abilities.
• Ability to work independently and thrive in a collaborative virtual team.

Required skills

• Proficiency with electronic health record (EHR) systems.
• Familiarity with virtual care tools.
