Registered Nurse (RN) – Care Coordinator

About the role

The Registered Nurse (RN) – Care Coordinator will join Coforge’s remote care management team in Muntinlupa, Philippines. The role focuses on triaging patient needs, coordinating multidisciplinary care, and delivering virtual assessments to support chronic disease management and preventive health.

Key responsibilities

• Document all patient interactions promptly in electronic health records (EHR) and care management platforms.
• Use clinical judgment to triage, coordinate, and escalate patient needs across physicians, pharmacy, laboratory, and radiology.
• Provide telephonic and virtual patient assessments, health coaching, and ongoing monitoring for assigned populations.
• Interpret clinical data, medical instructions, and test results to guide care plans.
• Handle vital signs and patient data in both hospital bedside and home‑health settings.

Required profile

• 6 months to 2 years of direct clinical experience in acute care, ambulatory care, home health, or care management.
• Proficiency with electronic medical records (EMR) and electronic health records (EHR) systems.
• Strong clinical assessment skills and understanding of chronic disease management.
• Ability to communicate effectively with multidisciplinary teams and patients remotely.
