Patient Care Coordinator (RN) – Remote with $10k Sign‑On Bonus

About the role

The Patient Care Coordinator (Registered Nurse) will support Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs for a U.S.-based client. Working from home, you will engage patients, track health data, and ensure clinical protocols are followed.

Key responsibilities

• Monitor patient health metrics (blood pressure, glucose, weight, etc.) via RPM platforms.
• Identify out‑of‑range readings and follow escalation protocols.
• Conduct structured outbound calls to engage patients in RPM and CCM programs.
• Reinforce physician‑directed care plans using clear, patient‑friendly communication.
• Provide compassionate support to elderly and chronically ill patients.

Required profile

• Active Registered Nurse (RN) license.
• Minimum 1 year of clinical, healthcare operations, or care‑coordination experience.
• Experience with chronic disease management (e.g., hypertension, diabetes).
• Background in structured patient outreach or communication activities.
