About the role
We are seeking a Virtual Patient Scheduler to support our home health services. This fully remote position involves direct interaction with patients and healthcare providers to ensure smooth admission processes.
Key responsibilities
- Handle phone inquiries from patients, families, and healthcare providers, answering questions about services, scheduling, and eligibility, and ensuring each call is documented accurately.
- Process referrals by reviewing incoming referral information, entering patient data into the scheduling system, coordinating with clinical teams to confirm service requirements, and tracking referral status through admission.
- Manage patient admissions for home health services by verifying patient eligibility, arranging initial home visits, updating admission records, and communicating admission details to relevant staff.
