Remote Care Coordinator – Philippines

About the role

The Care Coordinator supports patients throughout their healthcare journey by acting as the main liaison between patients, families, providers, and administrative teams. This remote position is based in the Philippines and focuses on ensuring seamless communication, timely service delivery, and adherence to care plans.

Key responsibilities

• Coordinate communication among patients, physicians, specialists, and multidisciplinary teams.
• Build, update, and monitor individual patient care plans and schedule follow‑up appointments.
• Maintain accurate electronic health records, process referrals, verify insurance eligibility, and respond to patient inquiries via phone, email, or chat.
• Connect patients with community resources, home‑care services, medical equipment providers, and financial assistance programs.
• Conduct routine check‑ins to monitor treatment adherence, address barriers, and educate patients on their care plans.
• Ensure all activities comply with HIPAA, patient privacy laws, and internal SOPs.
• Identify and escalate treatment delays, care gaps, or operational issues to clinical leadership.

Required profile

• Minimum 3 years of experience in US healthcare coordination, medical administration, patient navigation, or case management.
• Strong verbal and written communication skills with empathetic listening.
• Excellent time‑management, attention to detail, and ability to handle multiple care tracks.

Required skills

• Proficiency with Electronic Health Record (EHR) systems.
• Experience using HIPAA‑compliant communication tools.
• Microsoft Office Suite.
• Google Workspace.
