Patient Coordinator

About the role

SnappyCX is looking for a Patient Coordinator to join its clients’ care teams on a fully‑remote basis. The role acts as the primary liaison among patients, families, home‑care workers and other members of the care team, ensuring clear communication and high‑quality care delivery.

Key responsibilities

• Complete all required tasks according to agency guidelines and policies.
• Schedule work assignments based on Home Care Worker training and experience.
• Minimize the volume of open cases to ensure timely resolution.
• Document all communications accurately in HHA Exchange.
• Communicate effectively with patients, family members, contractors, clinicians, aides, and other departments.
• Provide necessary information to other healthcare professionals.
• Consult with families to ensure ongoing care and that client needs are identified and met.
• Advise HHAs of discharges, changes in service days/hours, or other relevant updates.
• Complete required daily, weekly, and monthly reports.
• Manage overtime and travel time requests.
• Utilize Smart Map and other tools to locate aides, provide directions, and check availability.
• Complete incident reports and manage mutual and linked cases.
• Respond promptly to emails, voicemails, and faxes.
• Adapt quickly to agency or industry changes and discuss concerns with the supervisor.
