About the role
The Community Care & Social Services Manager coordinates community‑based healthcare and social support services, ensuring individuals and families can access appropriate care and resources. Working closely with healthcare providers, social workers, government agencies and community organisations, the role supports care planning and promotes programs that enhance health and well‑being.
Key responsibilities
- Assess client needs and facilitate referrals to health and social services.
- Collaborate with healthcare providers, social workers, government agencies and community organisations.
- Monitor service delivery, maintain accurate records and address client concerns.
- Promote and implement programs that improve quality of life.
- Ensure compliance with organisational policies and relevant regulations.
