Clinical Reviewer (PHRN) – Remote (WFH)

About the role

ISTA Solutions is seeking an experienced Philippine Registered Nurse (PHRN) to work remotely as a Clinical Reviewer. You will audit patient charts, verify coding accuracy, and ensure documentation aligns with care plans.

Key responsibilities

• Review and audit patient charts and clinical documentation.
• Verify that ICD-10 diagnosis codes correspond with documented treatments and services.
• Evaluate patient care plans for alignment with diagnoses, prescribed treatments, and documented services.
• Complete structured audit reports containing over 140 questions per chart.
• Maintain high accuracy, consistency, and adherence to documentation standards.
• Achieve productivity targets by completing chart audits in roughly 30 minutes each.
• Identify and flag discrepancies, inconsistencies, or documentation gaps following established workflows.

Required profile

• Active Philippine Registered Nurse (PHRN) license.
• Familiarity with ICD-10 diagnosis codes.
• Experience reviewing or matching clinical documentation, treatments, and care plans.
• Experience with EMR tools, specifically Kantime.
• High attention to detail, strong analytical skills, and proficiency in Microsoft Office and web‑based forms.
• Good verbal and written English skills and residence within the Greater Manila Area; ability to work night shifts.
