Clinical Care Coordinator

About the role

We are looking for a Clinical Care Coordinator to provide remote support to a U.S.-based healthcare practice. The role bridges patients, providers, and insurers, ensuring timely and coordinated care. Ideal candidates have a clinical background and experience in care coordination or virtual medical assistance.

Key responsibilities

• Communicate with patients about appointments, treatment plans, follow‑ups, referrals and other care needs.
• Coordinate care among providers, specialists, pharmacies, and other healthcare professionals.
• Maintain accurate patient information and document interactions in the EHR/EMR.
• Assist with insurance verification, eligibility checks and prior authorizations.
• Conduct patient follow‑up and care‑management calls, including Chronic Care Management when required.

Required profile

• Clinical background as a Nurse, Physician Assistant, Medical Assistant or similar healthcare professional.
• Strong understanding of medical terminology and clinical workflows.
• Experience in care coordination, patient intake, or virtual medical support, preferably for U.S. practices.
• Excellent English communication skills; bilingual English/Spanish is a plus.
